Direct Care is a Prenatal and Child Care Coordination program. Direct Care assists pregnant moms and new mothers in maintaining a healthy lifestyle, along with their families, by providing resources. To qualify for the program you must be pregnant or have an infant up to 8 weeks of age, be enrolled in Medicaid or Badger Care Plus through the State of Wisconsin, and must live in Milwaukee County.
